Phase One Application WORD Action Plan Feedback Rubric (STARSS-LS) >> WORD | PDF When writing your action plans, there are seven key indicators that plan reviewers look for when providing feedback. This rubric shows the seven key indicators and a brief descriptor of what they mean. When applying for OETT/OK-ACTS Phase II Grants, these are seven of the eleven indicators used to evaluate the quality of the grant action plans. Action Plan Guidelines >> WORD | PDF The Action Plan Guidelines explain what needs to be included in an action plan, including the three narrative sections addressing supporting evidence, obstacles and action planning. One action plan including these sections is required to complete OK-ACTS Phase I. Three action plans are required for the OETT/OK-ACTS grant.
