Grant Application Instructions and Downloads

The OETT / OK-ACTS Phase II Grant Application is available as a PDF file, Microsoft Word document and in rich text format. Choose the one that is most appropriate for you. To use the PDF version, your computer must have the Adobe Acrobat software (not just Adobe Reader that you have downloaded free from the Internet).  The rich text version is needed for computers that use software such as Star office instead of Microsoft Office Word.  These instructions can be downloaded as a Word document by clicking here.

To download the grant application, complete the following steps:

• Click on the "Phase II Grant Document in the best format for you, (Word format or rich text) below.

• Select the "Save" button in the "File Download" menu to save the file to your computer.

• The application will be saved by default to either your Desktop or the "My Documents" folder on your computer.

Minimum Technology Equipment Specifications

The following procedures will assist you in completing the application:

• Remove Caps Lock.

• Fill in each blank beginning in the shaded box, then tab to the next form field.

• Many of the form fields have been created to limit the characterization space. You may not exceed the number of characters allowed within the field.

• When referring to page numbers, use the numbers in the lower right hand corner of the grant application pages.

• Be sure to save before you close the application.

• *This only applies to the PDF form: The first line total of the budget form should be filled in by hand to avoid copying the total to the second line quantity number.

To file your grant application, you will need to:

• copy the file to a CD (or floppy disk)

• print the file

• obtain the appropriate signatures

• make 5 copies

Submit the 6 copies (1 original and 5 copies) and the copy on the CD or floppy disk to:
