Memorandum of Understanding
I understand that there is a $299 registration fee and agree that the OK-ACTS laptop will become the property of my school district and added to its inventory. Furthermore, the laptop will be replaced if lost, stolen, or damaged. I also understand that attendance is required for the leadership seminar (2 days), cluster meetings (2 days OR four half-days) and online conferences. Furthermore, I understand that I am expected to work towards meaningful improvements in the targeted areas through networking activities and work with my school and district, for a total of 75 logged hours. I will complete required assessments associated with OK-ACTS in a timely manner. I will complete an action plan for my school and/or district, addressing a component of the High Achieving Schools rubric and using and integrating technology.

In applying for OK-ACTS, you are committing to the following requirements of the grant. To be considered for the program, this application must be completed with your supervisor's approval and received in the OK-ACTS office three weeks prior to the seminar starting. If accepted, your slot is non-transferable in the event you are unable to fulfill the requirements of the program.

Phase One Application

Please Note: Due to the large number of applicants, completing this form does not guarantee acceptance into the OK-ACTS program, nor does it guarantee you will receive the seminar date requested. You will be notified as soon as possible regarding acceptance and seminar dates.
